I just enabled only unlimited lives and I'm able to play the game. Did you downloaded a corrupted version of the file? If you enable just infinite lives you just get a constant firework on the title screen and can't play the game. If you enable Invincibility and it enters the attract mode you can't get to play the game. There's also crap text at the end of the doc.
