There is a basic version and a machinecode version at CSDb.
According to Rick himself on the Unepic SID Channel: "Yes, the first one was a one line basic program that read out an appended data array. It was written in Dec of 1981. The second was written some time later. That was 4 decades ago, so my memory is a bit fuzzy. If I remember correctly, the first one was played publicly in Italy at a C64 sales introduction there. At the time I was actually working for General Electric as the Integration Site Manager for the FA-18 flight control system in St Louis. I had some friends at CBM that had been trying to recruit me to work there. I did get involved with editing the 600 page C64 Programmers Manual (Reference Guide) as well as testing out some C64 hardware that CBM sent to me. Bob Yannes ask me to cobble up a quick music demo for his SID chip. I wrote "The Entertainer" in a couple hours and the rest is history. ;-) In 1986 I accepted a position at CBM as Test Engineering Manager. I worked mostly on the Amiga then. Fun times!"
