In the scroll text it says that the real name of Zaphod is Tom, so it is most likely this Zaphod: Zaphod so the demo is released around 20 March 1989.

Updated credits. This is a birthday demo (coded by Henning) with some lame scrolltext by Henning & Ela. Yes, it's in Norwegian.
