For the original RSI release: Mayhem in Monsterland +5. Can someone unlock this entry?

This is not the original release, it's an import by Spooks Factory.

I've clipped the name down, originally it was "Mayhem in Monsterland +5 Pal 1st Release". Certainly the last few bits have zero to do with the name of the entry.
